The problem

Our forms, records and PDFs sit across many spreadsheets. Staff chase files to fill one folder.

  • Forms, records and PDFs sat across many spreadsheets.
  • Staff chased files to fill one folder.
  • The paper layouts had to stay exactly the same.

What we built

  1. 18 structured forms. One web portal in place of the paper forms.
  2. Records on the right profile. Staff add and edit records against the right profile. Pick a patient and the form fills itself.
  3. The same layouts on paper. Two-page PDF output with signature areas.
  4. Digital signatures. Signatures are captured and embedded in the PDF.
  5. Roles and two-step login. Staff and admin roles, each seeing only its own data.
  6. Yours to keep. Works on phones. Code and database handed to the owner.

What changed

Delivered and live, and the milestone is approved.

Paper intake is gone, and the printed forms keep the layouts staff already know.
